If you live in our practice area (Burham, Eccles, Wouldham and Blue Bell Hill) and would like to register with us, please complete one of our registration forms that are available from our reception desks or downloadable below.

Please note that you will be registering with the practice rather than an individual GP although all patients within the Practice will have a named, accountable GP.

The surgery also has a range of clinics and for further details please see reception.
